Course Description
Transitions of care are pivotal moments in a resident’s life, where they move between different healthcare settings, often resulting in complexities and risks that require careful management. High-quality transitional care is especially important for older adults with multiple chronic conditions and complex therapeutic regimens and the peace of mind of family members.
A growing body of evidence suggests that older people are particularly vulnerable to breakdowns in care and thus have the greatest need for transitional care services as poor communication, hand over and documentation has been linked to medication errors, low satisfaction with care, and high rehospitalization rates.
